Please indicate your understanding of the submission process and the expectations of Non-Profit Membership use.

Submission of this application in part or full does not guarantee access to the Louisville Zoo or a Non-Profit Membership. Additionally I understand that if my application is accepted:*

1. our Non-Profit Membership is intended for use by clients of my specific organization/location and the staff supporting them. 2. each location or branch of my organization requires its own separate Non-Profit Membership 3. my organization's Non-Profit Membership is intended to be used for our clientele and is not meant for personal use by any employee, volunteer or other representative of the organization 4. anyone representing our organization will refrain from using or sharing our Non-Profit Membership with employees, friends or family for personal use or personal gain 5. anyone representing our organization will be required to show proof of employment (an employee id, name tag or equivalent) plus one of the provided membership cards for admission, and admission will not be granted without them 6. the total number of staff and clients cannot exceed 15 people (up to 5 staff and 10 clients) during a single visit and we will be required to purchase tickets for anyone in excess of the permitted guests 7. the Louisville Zoo reserves the right to refuse admission to anyone unable to meet the expectations outlined above 8. it is our responsibility to share the rules, requirements and expectations written or implied of our Non-Profit Membership with any and all staff and clients
